Working together

Intrinsic Care works closely in partnership with a numbers of local council, Community Mental Health Teams and other professional organisations in order to maintain the best possible service provision which is in-line with expectation and needs, including NHS Trusts we serve. Our aim is for every individual we support to have the opportunity to achieve the best possible outcomes and quality of life and offer value for money to funding bodies.

We utilise our experience and expertise to develop and maintain long lasting relationships, building trust and affordable / proactive solutions, whilst offering a transparent service of integration innovation and clinical effectiveness to ensure that we become the provider of choice.

Intrinsic care believes that working in partnership with professionals is a constantly managed process, meaning that we value the relationship we build with you so we can demonstrate to the people we support that we share responsibility for them. Also showing that we can work together in their interest to make it possible and provide the best possible future, outcomes and quality of life.

By working closely with the organisations, we understand their needs and can adapt our business when and where appropriate – whether this is evolving existing services or creating new ones.

AT A GLANCE

REFERRAL

Acceptance to our High Supported Accommodation is through referral to our dedicated Managers using the contact details below. For each referral we will conduct:

A review of the applicant’s current presentation, historical needs and circumstances.

An assessment related to the composition of the scheme’s current residents.

An assessment as to whether the applicant meets the criteria.

Emergency admissions will be accepted, provided that the above information has been supplied to consider the appropriateness of the placement.

Intrinsic Care reviews each person’s placement plan on frequently. This is undertaken through consultation with the service user and representatives from the mental health team to ensure that the plan is current, appropriate and relevant to the service user’s needs. Any significant change to the placement plan will be discussed and agreed at a formal review.
